Honored Citizen Card Requirements
To better serve the passengers that ride on the SETD
buses, SETD will be requiring all passengers who would like to receive a
discounted fare on "THE BUS", to fill out and return an application for an
"Honored Citizen ID Card".
Anyone with a valid Medicare card is
automatically an SETD Honored Citizen and may show his or her Medicare card to
ride the bus for Honored Citizen reduce fare. SETD personnel may ask to see
other valid proof of the rider's identity. Persons who are under 65 and
CANNOT be verified disabled through any of the following agencies
may apply for an SETD Honored Citizen ID Card.
- Commission for the Blind,
- U.S. Department of Veterans' Affairs,
- Vocational Rehab,
- Senior and Disabled Services,
- US Railroad Retirement Board or the
- Social Security Administration
This card is for anyone who is able to use regular bus service, is
under 65 years of age and are verified disabled by a physician licensed to
practice medicine under the laws of the State of Oregon or Washington according
to the following definition:
"A person must have a permanent impairment due to
illness, injury, congenital malfunction or other incapacity or disability that
substantially limits one or more of that person's major life activities.
"Permanent Impairment" means an impairment which has lasted (or is expected to
last) at least 12 months."
The applications are available from all
Drivers and from the SETD Office at either 900 Marine Dr., Astoria, OR. or 465
N.E. Skipanon Drive, Warrenton, OR. 97146. It is the applicant's responsibility
to complete the application and have it signed by a duly licensed physician.
SETD is not liable for any expense. SETD reserves the right to require proof of
disability in addition to this Physician's opinion.
